At Winchester Family Medicine, the healthcare providers, Tyler Campbell, M.D. and Becky Basford, CNP (Certified Nurse Practitioner), provide a broad range of healthcare services for your family. They work with you to ensure your continued good health and help you understand your health conditions, treatments and medications.
At Winchester Family Medicine (WFM), we are very focused on service and strive to keep office wait times to less than 60 minutes.

APPOINTMENTS:
All patients are seen by appointment. Appoints may be made during office hours. If an acute problem arises, please try to call the office before 10:00 a.m.(noon on Thursday) so that we may arrange to see you that day. We try to stay on schedule to avoid long waiting times.
There are several things you can do to help us stay on schedule. When calling to schedule an appointment, be specific about the nature of your visit. A “routine appointment” is not designed to exceed fifteen minutes. If you call for an appointment that you anticipate may necessitate more than 15 minutes, please indicate this to the receptionist when scheduling your appointment.
Please call 24 hours in advance if you must cancel an appointment.
Please attempt to be on time for your appointment so others will not have to wait. If you are late, your appointment may have to be rescheduled.
Please understand that because we do try to accommodate emergencies, there are times when we are running behind schedule. We ask for your understanding and patience during these times.
CONSULTANTS:
As a Family Practice, we are trained to handle 90-95% of all problems seen in our office. However, we utilize specialists in complicated situations. We offer continuity of care and help in interpreting and coordinating the specialist’s advice. If you desire a second opinion, we will help arrange this.
CO-PAYS AND OUTSTANDING PATIENT BALANCES:
Co-pays and any outstanding balances are due at your appointment.
We accept cash, check, debit and the following credit cards, American Express, Visa, MasterCard, and Discover.
Co-pays are set by your insurance company and are considered a contract between you, your insurance and employer. If you have questions regarding this please contact them.
Insurance:
• Please verify that you have designated Dr. Tyler Campbell or Becky Basford, CNP as your primary care provider,if
your insurance requires you designate a primary care provider.
• In order to serve our patients, we are contracted with many insurance carriers. However, it is your responsibility to know
your benefits and coverage.
• Our office will bill your insurance but ask that you follow-up if claims are unpaid for any reason.
• If your insurance plan requires that you use their claim form or if you prefer to file your own insurance claims, we will
provide the information necessary for you to file directly to your insurance company.
• Not all services that your healthcare provider recommends may be covered by your plan, so it is vital for you to check
your benefits and coverage.

Self-Pay Patients:
We require payment at the time of service from our self-pay patients. We can give you an estimate of charges prior to your appointment but we will not know the exact charge until after you have been seen.
Acceptable Methods of Payment:
We gladly accept cash, personal checks, credit cards and bank cards. If a check is returned from the bank, you will be assessed a $25.00 fee. You will be required to pay cash or by credit care for all future checks if your check is returned from the bank.
Laboratory Tests:
A few laboratory tests are performed at WFM. If a lab test is performed at WFM, the charge for that test will be included in your WFM bill
Most laboratory testing is performed at Adams County Regional Medical Center (ACRMC). If you have laboratory testing that is referred to ACRMC, your testing will be billed to your insurance company. We will make every effort to assure tests are covered by your insurance. However there are times when certain tests are not covered and you may be requested to sign an Advanced Beneficiary Notice (ABN).
Medical Record Copying Fees:
There is a fee for copying and mailing medical records. Our charge is the allowable amount per guidelines set from the State of Ohio.
