Social Services are provided to inpatients, outpatients and employees. Referrals
are accepted from physicians, hospital personnel, patients and families, outside
agencies and others. Patients considered high risk for needs are evaluated for
possible services.
Services include:

Patient evaluations
•
Situational counseling
•
Discharge planning
•
Assistance with Advance Directives
•
Liaison with agencies in the community
Services to Home Health Care are also provided. The Social Worker works closely
with the interdisciplinary team to develop a holistic discharge plan with the
patient and patient’s family.
