In-patient Clinical Nutrition Therapy:
Clinical nutrition services are available for in-patients during their hospital
visit. Nutrition consults can be requested from identification of risk for
nutritional problems on admission, via a physician order or as an order from other
healthcare team members.
Out-patient Medical Nutrition Therapy:
Medical Nutrition Therapy (MNT)
With MNT an individualized nutrition plan is developed by a registered licensed
dietitian to meet the patient’s specific
needs. MNT covers nutrition assessment of the patient,
nutrition diagnosis, and nutrition intervention including diet
therapy and diet counseling/education.
Physician/Health Care Provider Referral (Doctor’s Order)
Before an appointment can be scheduled a health care provider’s
referral, written doctor’s order, is required. The
written order must include diagnosis and supporting/related medical
information.
Appointment
Once your health care provider has provided the dietitian with a written order for
MNT, you may call (937) 386-3861
to schedule your appointment. You may need to leave a message
if the dietitian is not available when you call. You
will receive a return phone call to then schedule your appointment.
Payment
Please check with your insurance company before you schedule
your appointment. Your insurance company may or
may not cover all the cost of MNT. Medicare covers MNT for
patients with either a diagnosis of diabetes or a diagnosis
of renal (kidney) disease.
If you do not have insurance we may be able to help you apply for
financial assistance. You may call 937-386-3449,
937-386-3450 or 937-386-3451 to ask about financial assistance.
You may call 937-386-3861 if you would like more information.
